Volunteer Authorizations

Volunteer Liability Waiver and Agreement:

The undersigned volunteer, hereby acknowledges and agrees to the following terms and conditions:

1. Assumption of Risk: I understand that volunteering involves inherent risks, including but not limited to physical injury, illness, or property damage. I voluntarily assume all such risks associated with my participation in PrismaCorp Foundation activities.
2. Release and Waiver: In consideration of being allowed to volunteer for PrismaCorp Foundation, I hereby release, forever discharge, and hold harmless PrismaCorp Foundation, its officers, directors, employees, and agents (collectively referred to as the “Released Parties”) from any liability, claim, demand, and action whatsoever arising from or related to my participation in volunteer activities. This includes any transportation, first aid, assessment, care, treatment, response, or service rendered during my volunteer work.
3. Medical Authorization: I authorize PrismaCorp Foundation to seek medical treatment for me in the event of an emergency. I understand that reasonable efforts will be made to contact me or my emergency contact before any such treatment is administered.
4. Photographic Release: I grant PrismaCorp Foundation the right to use photographs or videos of me taken during volunteer activities for promotional or educational purposes.
5. Force Majeure and Acts of God:
   • In the event that volunteer activities are disrupted, postponed, or canceled due to circumstances beyond the control of PrismaCorp Foundation, including but not limited to natural disasters, extreme weather conditions, civil unrest, or government actions (collectively referred to as “force majeure” or “acts of God”), I agree not to hold the Released Parties liable for any resulting damages or losses.
   • I understand that force majeure events may necessitate changes to volunteer assignments, schedules, or locations. PrismaCorp Foundation will make reasonable efforts to communicate such changes promptly.
6. Volunteer Agreement: By signing below, I affirm that I have read and understood the terms outlined above. I agree to abide by PrismaCorp Foundation’s policies and guidelines while volunteering.
